Version 2026-09-26-v2-draft
Target Number Attestation
DRAFT, not legal advice, to be reviewed by counsel. This file is the reference text of the attestation that an authorized user of a customer organization signs in the application before a target number can be called in the sandbox. The application stores, for each signature, the text version below, the user (id, name typed as signature, email), the organization, the target number, the date and time (UTC) and the IP address. A new version must get a new identifier and be added to the history below; signed versions must never be edited.
How the attestation is used
- It is signed once per target number, before the first call to that number.
- Alone, it opens only the sandbox: 3 test calls per number and per 24 hours, one at a time,
3 minutes maximum, manual runs only, between 8 am and 9 pm local time of the number
(source:
app/qa/verification.pyconstants anddocs/public/number-verification.md). Full use requires a successful ownership verification. - It must be signed again when a new text version becomes mandatory, or when the number is deleted and registered again.
- It is shown in full on screen and in the API documentation, with a checkbox that is not pre-checked, and a field where the signer types their full name and title.
[COUNSEL: confirm that a typed name plus checkbox, with the metadata listed above, is
sufficient as an electronic signature for this purpose under the chosen governing law (in the
EU, a "simple electronic signature" under Regulation (EU) No 910/2014 art. 3(10) and art. 25;
in the US, the E-SIGN Act, 15 U.S.C. § 7001), and whether the signer's title is needed.]
[PRODUCT: decide whether only Admins may sign (current code also allows Members). The text
below is written so that any authorized user can sign on behalf of the organization.]

Attestation for automated test calls to {phone_number}
I, {signer_name}, confirm that I am authorized to act on behalf of {organization_name} (the "Organization") and I certify, on behalf of the Organization, that:
- The Organization is the subscriber of the phone number {phone_number}, or holds a prior written authorization from its subscriber to have it called by automated systems using artificial or synthetic voices, for testing purposes. The Organization will keep that authorization and provide it to CallQA on request.
- This number is not an emergency number, a premium rate or special number, or the personal line of a person who has not agreed in writing to receive these calls.
- The calls will only be used to test the telephone system that answers this number. They will not be used for marketing, sales, surveys, or to reach any third party.
- Where a person may answer or listen to these calls, the Organization has informed that person in advance and obtained any consent required by law for automated calls, for the use of an AI voice, and for the recording and transcription of the calls.
- The calls, their content and any test data comply with the laws that apply to them, including laws on automated and artificial voice calls (such as the US Telephone Consumer Protection Act), telemarketing, call recording and data protection, and with the CallQA Terms of Service and Acceptable Use Policy.
- If the Organization stops controlling this number, it will stop calling it and remove it from CallQA without delay.
I understand that CallQA relies on this attestation to place the calls, that it records my name, email, IP address and the date and time of this signature as evidence, that it may suspend calls to this number at any time, and that the Organization is responsible for any claim resulting from calls placed at its request, as set out in the Terms of Service.
Fields:
- {phone_number}: the E.164 number of the target, as registered.
- {organization_name}: the organization's name at the time of signature.
- {signer_name}: the full name typed by the signer.
Checkbox label: "I have read and agree to this attestation on behalf of {organization_name}."
